If you live or move within our practice area, and are eligible to receive NHS treatment, you can register at the surgery.
You will need to collect a registration form and a new patient medical questionnaire from reception. These should be completed and signed and returned to reception.

Please note, when completing patient information for children under 6 years of age, we need details of their immunisation history.
When you return the completed forms, please bring a means of photo identity such as a passport or a driving licence and a proof of address such as a recent utility bill or bank statement.
If you are on regular medication, please inform us by bringing in your last prescription slip.
Please let us know of any change in your details such as change of name, address and/or telephone number. If you move house you may need to change doctor if you are out of our catchment area.
Please check with the receptionists to find out if you are still in our area.
If you have moved outside of our catchment area, you can remain as a patient at our practice. Please be advised that you may need to travel to our practice if we require to see you in person.
