If you live or move within our core practice area (see map below), and are eligible to receive NHS treatment, you can register at the surgery.

Thameside Medical Practice now welcomes patients who live outside our core catchment area!

This can be helpful if you:

· want to use a GP surgery near your work instead of your home

· are already registered with our practice but have recently moved away from the local area

· prefer the services of a GP surgery that’s further away

In order to register with us if you live outside of our catchment area, you must:

– Be aged over 18

– Be able to travel to the surgery independently for appointments when necessary

– Understand that you may not be eligible to access some community-based services, e.g. home visits, Community Nursing Team

Please note, when completing patient information for children under 6 years of age, we need details of their immunisation history.

If you are on regular medication, please inform us by bringing in your last prescription slip.

Alternatively, you can collect an online patient access form from reception.

Before we can grant you online access, you MUST provide us with a form of photo ID (i.e., your drivers license) and proof of your current address (i.e., a bank statement). You can either bring ID into reception for us to take a scanned copy. Alternatively, once you have completed this form, we will send you a text with a link for you to upload a photo of your ID.
